Tier 1 Services (55+ years)

• Annual comprehensive physical with review of medical records and history.

• Develop plan of care and goals for health maintenance.

• Comprehensive labs: cholesterol panel, blood count, thyroid, chemistry panel, urine testing, Vitamin D,  (PSA or Pap testing as needed).

• EKG and office-based chest x-rays also included, if needed.

• Yearly flu shot.

• Hospitalizations, including admission history and physical, daily hospital visits through discharge, and initial post hospitalization office visit.

Tier 1 Add-On Services:

• TDAP (tetanus + whooping cough) $50

• Tetanus booster  -  $25

• Discounted prices on select medications for urgent care

• Access to discounted prices for many common imaging services

Tier 2 Services (Age 41-54)

• Annual comprehensive physical with review of medical records and history.

• Develop plan of care and goals for health maintenance.

• Comprehensive labs: cholesterol panel, blood count, thyroid, chemistry panel, urine testing, Vitamin D,  (PSA or Pap testing as needed).

Tier 2 Add-On Services:

• EKG $20

• Office-based chest x-ray $50

• Yearly flu shot $25

• Annual comprehensive physical with review of medical records and history.

• Develop plan of care and goals for health maintenance.

• Comprehensive labs: cholesterol panel, blood count, thyroid, chemistry panel, urine testing, Vitamin D,  (PSA or Pap testing as needed).

Tier 3 Add-On Services:

Tier *4 Services (age 16 to 26)

*This tier is only applicable to person(s) with at least one parent/guardian currently enrolled at Total Primary Care.

• Annual comprehensive physical with review of medical records and medical history

• Comprehensive labs: cholesterol panel, blood count, thyroid, CMP (chemistry panel), in-house urine

testing (if needed), Vitamin D, PSA or Pap (if needed)

• Develop plan of care and goals for health maintenance

Tier 4 Add-on services:

• Tdap vaccine - $50

• TD vaccine - $25

• EKG - $20

• Spirometry - $20

• Office based chest x-ray - $50

• Nebulizer treatment - $15

• Yearly flu shot - $15 (subject to change based on market cost)

• Hospitalizations (at Menorah Medical Center) - Hospital charges will be a flat rate of $250 for each

hospitalization regardless of how many days hospitalized

Financial Responsibility

Total Primary Care is non-insurance and non-Medicare based, therefore it is the responsibility of the individual participant to pay the yearly membership fee for the program.  Fees are paid on a quarterly, semi-annual or annual basis by either check or credit card.  For those with traditional insurance, we will provide one coded insurance claim form (after your physical) so that some reimbursement might be possible from your insurer.

Please remember that tests done outside of our office are NOT COVERED by this agreement and will be billed directly by the facility performing the service.  Advanced lab testing (not considered routine) will be billed to traditional insurance.

YOU ARE ENCOURAGED TO CARRY INSURANCE COVERAGE FOR NON-COVERED SERVICES.

Enrollment

Membership enrollment begins the date your signed contract is received and will be renewed annually on that date.

Memberships will be accepted until we have reached the number of enrollees that Dr. Ponnuru feels he can manage while providing the best comprehensive and personalized services.

Termination

A participant may terminate their membership at any time with a 30 day written notice.  Upon termination, fees paid will not be refunded after the annual physical has been completed or if hospital care was provided.  THE PATIENT IS RESPONSIBLE FOR THE BALANCE OF THE YEARLY FEE, IF NOT YET PAID.
